- All patients under the age of 18 MUST be accompanied by an adult to the office.
- Please complete any patient demographic information on the form we send you, or if we didn’t send you anything, please go under FORMS / DOWNLOADS and print out / fill out a patient demographic form.
- Please bring your insurance card with you to the appointment.
- Please bring any CONTACT LENS boxes or foil packs so we know what brand/model/power of contacts you are wearing. This will save you a lot of time here at the office.
- If you would like us to review old records from a previous eye care provider, please allow enough time for you to request a release of information (ROI) sheet FROM your referring provider, sign it, and get it TO our office, so that we can track down your old records. No office will send your records to us without you signing a release of information form for their records. Like it or not, that is a HIPAA requirement. Not a rule of our liking, but a rule we must obey.
- Check and see if your insurance requires a “REFERRAL” to come see us. You would get that from the doctor that referred you to us. If your insurance company requires a REFERRAL and if we don’t have it at the time of your appointment, your best choice is to reschedule until after you obtain one. Your insurance company will never “back-date” a referral for a visit that happened already. If you do not wish to reschedule, you will need to be seen as a “self-pay” patient and payment will be expected at time of service by cash or credit card. Our advice, if you cannot obtain a referral before your visit, is to go the “reschedule” route so you are not on the hook for a big medical bill. Waiting for a referral to come in while you are waiting at the front desk is usually pointless and you might end up rescheduling anyway. CHECK AND SEE IF YOUR REFERRAL HAS ARRIVED AT OUR OFFICE AT LEAST 2 DAYS PRIOR TO YOUR APPOINTMENT TIME. CALL US (540) 681 - 1211 IF YOU ARE NOT SURE IF WE HAVE RECEIVED IT YET. We work with hundreds of insurance plans, each with thousands of different rules, so we cannot know your individual insurance requirements. YOU are responsible to know whether you need a referral or not and to know how YOUR insurance works.
- Deductibles and Co-pays: Many forms of insurance coverage require a co-pay and/or deductible to be collected from the patient. Our office policy is to collect your co-pay or deductible at the time of your visit.
- You will be charged a $35 fee for NSF bank fees/returned checks (which is what we get charged by the bank).
- CANCELLATION / Cancelled appts / MISSED APPTS: If you are unable to make an appointment, please notify our office at least 24 hours in advance so that your visit time may be rescheduled and your unused appointment time offered to another patient. You may incur a $45 fee for habitual missed appointments if we are not notified in advance. A strong history of no-show appointments (greater than 3) may result in termination from our practice. Please cancel your appointment if you are not going to show for your appointment so we are not sitting there waiting for you and can offer your time to someone on our waiting list. Similarly, if you no-show to your pre-scheduled surgery date at the hospital or surgery center, you may be charged $250.
